    Environmental Implications of the Health Care Service Sector

    This report analyzes the environmental effects associated with activities undertaken and influenced by the health care service sector. It is one part of a larger study to better understand the environmental effects of service sector activities and the implications for management strategies. Considerable analysis has documented the service sector's contribution to domestic economic conditions, yet little analysis has been performed on the broad impacts service firms have on environmental quality. For this study the authors developed a framework to examine the nature of service sector industries' influence on environmental quality. Three primary types of influence were identified: direct impacts, upstream impacts, and downstream impacts. In addition, indirect impacts induced by service sector activities include their influence over settlement patterns and indirect influences over other sectors of the economy. In their initial analysis, the authors noted that many functions performed in the service sector also are commonly found in other sectors. The impacts of these activities have been analyzed separately from those unique to the health care sector, as they present different challenges. Health care is one of the largest U.S. industries, employing one in nine workers and costing one in seven dollars generated in the economy. Functions performed in the industry that are common in other sectors include: transportation; laundry; food services; facility cleaning; heating and cooling; and photographic processing. Activities unique to the health care industry include: infectious waste generation and disposal; medical waste incineration; equipment sterilization; dental fillings; ritual mercury usage; x-ray diagnosis; nuclear medicine; pharmaceutical usage and disposal; and drinking water fluoridation. The industry has considerable leverage upstream on its suppliers, which is important to managing risks from the use of goods commonly used in the industry, including: mercury-containing products, polyvinyl chloride plastics, latex gloves, and syringe needles. The authors identified a number of areas for potential environmental management initiatives: controlling emissions from on-site "production" type functions; mercury use; the environmental consequences of infection control measures; pollution prevention through substitution of alternative health care services; and research and data collection.

    Not In My Back Yard, or NIMBY, in its various forms, has three principal types of targets. The first is waste disposal facilities, primarily landfills and incinerators. The second is low-income housing. The third is social service facilities, group homes and shelters for individuals such as the mentally ill, AIDS patients, and the homeless. This Article addresses the issue of the victims of NIMBY, with special reference to the effects of project opposition on racial minorities. Because the effect of facility opposition varies widely with the type of project involved, Part II arrays the types of relevant projects and shows the ways that opposition manifests itself. Part III then briefly discusses the legal techniques used by those who oppose facilities, and the counter measures used by facility proponents. Part IV examines the available evidence on who suffers as a result of the opponents\u27 techniques. Part V looks at who benefits from opposition to siting new facilities. Part VI is devoted to some of the secondary and imponderable effects of facility opposition. Finally, Part VII draws conclusions from the preceding discussion, and shows how the costs and benefits of NIMBY are very different from those envisioned by those who either condemn or applaud facility opposition

    Space augmentation of military high-level waste disposal

    Space disposal of selected components of military high-level waste (HLW) is considered. This disposal option offers the promise of eliminating the long-lived radionuclides in military HLW from the earth. A space mission which meets the dual requirements of long-term orbital stability and a maximum of one space shuttle launch per week over a period of 20-40 years, is a heliocentric orbit about halfway between the orbits of earth and Venus. Space disposal of high-level radioactive waste is characterized by long-term predictability and short-term uncertainties which must be reduced to acceptably low levels. For example, failure of either the Orbit Transfer Vehicle after leaving low earth orbit, or the storable propellant stage failure at perihelion would leave the nuclear waste package in an unplanned and potentially unstable orbit. Since potential earth reencounter and subsequent burn-up in the earth's atmosphere is unacceptable, a deep space rendezvous, docking, and retrieval capability must be developed

    The European waste sector is undergoing a period of unprecedented change driven by business consolidation, new legislation and heightened public and government scrutiny. One feature is the transition of the sector towards a process industry with increased pre-treatment of wastes prior to the disposal of residues and the co-location of technologies at single sites, often also for resource recovery and residuals management. Waste technologies such as in-vessel composting, the thermal treatment of clinical waste, the stabilisation of hazardous wastes, biomass gasification, sludge combustion and the use of wastes as fuel, present operators and regulators with new challenges as to their safe and environmentally responsible operation. A second feature of recent change is an increased regulatory emphasis on public and ecosystem health and the need for assessments of risk to and from waste installations. Public confidence in waste management, secured in part through enforcement of the planning and permitting regimes and sound operational performance, is central to establishing the infrastructure of new waste technologies. Well-informed risk management plays a critical role. We discuss recent developments in risk analysis within the sector and the future needs of risk analysis that are required to respond to the new waste and resource management agenda

    Cleaning Up the Nuclear Weapons Complex: Exploring New Approaches

    In recent years, policy experts have increasingly decried the "piecemeal" approach to environmental protection embodied in existing laws and regulations. This paper analyzes one aspect of the call for more integration: the feasibility and advisability of developing an integrated approach to regulating the cleanup of the nation's nuclear weapons complex. The Cold War has left an unprecedented set of difficulties at sites across the country where vast quantities of hazardous and radioactive materials must be properly managed. Regulatory fragmentation�particularly the phenomenon of multiple regulators and regulations driving the allocation of resources in an uncoordinated fashion�is nowhere more evident than in the current statutory and regulatory framework governing environmental management activities at this array of sites�the nuclear weapons complex. The objective of an integrated approach is to give regulators and the regulated community incentives to look holistically at environmental hazards, both existing and future, and to develop creative ways of setting priorities such that risk, cost effectiveness, and public concerns are taken into account. This paper describes recent efforts to achieve integration in the environmental arena, surveys the unique technical, regulatory, and political circumstances surrounding cleanup of the weapons complex, and offers some preliminary thoughts on how integration might be attained in this vital area. While administrative and regulatory changes could achieve some important steps in this direction, the authors conclude that legislative change is necessary for implementation of a truly integrated approach.

    This report summarises the lessons to be drawn from the three-year collaborative social sciences research project ‘International Socio-Technical Challenges for implementing geological disposal’ (InSOTEC). Adopting an approach that is relatively novel in this context, the project focused its investigations on the complex interplay between what are typically seen as distinct technical and social dimensions of radioactive waste management (RWM), in particular in the context of the design and implementation of geological disposal. The aim of the InSOTEC project was not to arrive at a prescription for facilitating the implementation of geological disposal, but to foster and deepen the growing awareness of the interaction between social and technical aspects of RWM that has been evident within the technical expert community by providing stakeholders and experts of all kinds with a better understanding of the processes that shape the challenges which confront them. The report brings together insights for RWM that have been generated within the different research strands of the project and offers observations on their implications for practice, addressing in particular the processes of research and development, public and stakeholder involvement in RWM, and long-term governance of geological disposal of higher activity radioactive wastes
